Doctor can't be trusted. Pretended to have seen me in a psychiatric hospital when I had NO visit with him. He even went as far as creating exam notes that when I received them were very inaccurate. He had made up some of it claiming I had some conditions which I have never had and mixed it with a prior visits notes. A complaint was filed and he was disciplined.
