Dr. Lear has performed over 30,000 Mohs surgeries and has been active in research, development and teaching throughout his career. In 2016, he and his partner founded SUTUREGARD Medical and commercialized HEMIGARD wound closure devices to address the problem of dehiscence, which is the re-opening of a closed surgical wound. Clinical studies have shown that HEMIGARD devices reduce dehiscence in multiple specialties (dermatology, Mohs, orthopedic surgery) by over 80%. In 2017, Dr. Lear founded the Samaritan Health Services Micrographic Surgery and Dermatologic Oncology (MSDO) fellowship program (one of two such fellowships in the Pacific Northwest) in which he trains one board certified dermatologist per year on Mohs surgery and advanced facial and non-facial reconstructions using flaps, skin grafts and other modalities. He has a fourfold mission to (1) ensure patient safety, (2) enhance patient comfort, (3) optimize value-based care and (4) educate his fellow dermatologists and Mohs surgeons. During Mohs surgery with Dr. Lear, patients may have one or two loved ones remain with them in a dedicated room for the entirety of their procedure (removal and reconstruction). This practice avoids problems and costs that can be associated with moving Mohs patients to different rooms during their procedures. Dr. Lear has developed a unique and rigorous Mohs Quality System that involves the surgeon personally and expertly grossing and inking (key elements of Mohs surgery) every Mohs specimen removed from a patient. This practice reduces the potential for error in the Mohs laboratory and is a vital step to enhance patient safety.
