It is one of his usual, traditional, generic parting gifts he gives, so he can make a quick exit and move onto his next order of business for himself, because it is only himself he cares about. This doctor works all the time because he has to. He is an absent father and an absent husband. He is not a good doctor or surgeon. He has to support children he has had outside of marriage and he hides them. He has never been faithful to a woman in his life.
