This doctor owns his own part of Gateway Surgical Center and was not willing to do my surgery at Phoenix Baptist Hospital where I have 100% coverage (states he goes to out pt and PBH on website) but instead only wanted me to goto Gateway and pay an out of network $750 copayment.
